 Please read the contract terms below carefully and in full.

Set-Up/Break-Down:

Set-up will be early as Thursday 19th, 5-10pm. Friday, September 20th from 8am till 12pm. You must be completely set-up by 12pm for the health department to do there inspection Friday morning.

 Booth Reservation & Important Info:

    • Each booth includes: 8x8’ space, (2) vendor passes, (2) tables, and (2) chairs.

    • All booth reservations/locations will be made on a first come, first serve basis, with payment in full.

    • No refunds on payments.

Single booths(8'x8') are $400.00. You may purchase up to 2 of these

OPEN to the public:Friday Sept 20th, 2019 , 2:00pm-10pm

Saturday Sept 21, 2019 (10:00am – 10:00pm)

Sunday Sept22, 2019 (10:00am – 6:00pm)

ALL ARTIST STEP 1.

 There will be a Health Department $100.00/per artist fee for licensing. 

Fill out Wake CountyTattoo  permit application.

Deadline for permit completion is Oct 15th.

 

STEP 2. (Follow this link to create account for temporary licensing credit card billing)

Each Artist must create an account here. Once your account is created , and permit application has been received, Wake County will issue an invoice payable via credit card. Please wait several days for application to be processed. Once it is processed your portal account will show $100 fee now payable.

        

    

By filling out the form below, you agree to an electronic signature, and to attend the 2019 Tattoo Fest. Once your registration form is received an invoice will be emailed back to you, from 401 Tattoo and Art.
